About a MACS Lift

A MACS lift lifts both the skin and the underlying supportive tissue of the cheeks. This helps reduce sagging, deep folds around the mouth, and so-called “hamster cheeks,” while giving the jawline more definition. During the consultation, the plastic surgeon will discuss whether a MACS lift is suitable for your face, the degree of skin sagging, and your desired outcome.
  • A MACS lift costs €5,500. Depending on your wishes, the treatment can be combined with, for example, an eyelid correction, forehead lift or liposuction of the double chin. During a free, no-obligation consultation, the plastic surgeon will discuss the options and you will receive a transparent cost overview.
  • In a MACS lift, the skin of the cheek and the underlying supporting tissue are lifted, but the neck is not included. A full facelift is more extensive and also treats the neck. Which treatment suits you best depends on factors such as the degree and location of the skin laxity and the result you want.
  • After a MACS lift, your face may be bruised, sore and swollen. The swelling and bruising may increase during the first three days and then gradually subside. Allow a recovery time of approximately 7 to 14 days. The result becomes increasingly visible as the swelling subsides and the tissue continues to heal.
  • At Boerhaave, a MACS lift is performed by experienced plastic surgeons with an eye for safety, quality and a natural result. You can count on personal guidance and extensive aftercare, so that you know where you stand throughout the entire treatment process.

  • Facelift Options

The facelift comes in several variants that differ in terms of design and cost.
You can think of it as a three-step process:

    1. S-lift: This is the basic procedure. Through an S-shaped incision, only the skin of the cheek is lifted. The underlying supporting tissue is not lifted.
    2. MACS lift: Using the same incision, the skin of the cheek and the underlying supportive tissue are lifted together. The neck is not included.
    3. Facelift: Through an extended incision behind the ear, the skin of the cheek, the underlying supporting tissue, and the neck are lifted.

    • A MACS lift is always performed under general anesthesia. This means you’ll stay overnight at our clinic afterward so that your blood pressure and heart rate can be monitored. If you’re feeling well the next morning and the plastic surgeon deems it safe, you may go home under supervision.

Before the general anesthesia is administered, the plastic surgeon will mark the area where the procedure will take place. An incision will then be made starting at the temple along the hairline and extending downward in front of and behind your ear.

This allows the skin and the layer of connective tissue between the skin and muscles to be released. First, the connective tissue will be repositioned to its original location. The tissue will also be shortened by pleating it—that is, by folding it in half and suturing it to the underlying layers. This will give you more volume around your jawline.

After the excess tissue has been removed, the skin in front of and behind the ear is sutured with a dissolvable suture.

Because a strip of skin has been removed, tension is created. This creates a risk that your scars may not heal as nicely. That is why plastic surgeon Artiss uses skin adhesive. By first bonding the underlying tissue, there is less tension on the sutures, and the scar heals with a visibly better appearance.

Another product that can be used is Tisseel, which ensures effective hemostasis. This means that minor bleeding is stopped, reducing the risk of post-operative bleeding. The airtight bonding also reduces tissue fluid buildup (edema), leading to better recovery and a faster final result.

Thanks to this adhesive, patients no longer need drains or pressure bandages. Only a headband is needed to protect the wound.

  • After the surgery, you may experience a mild, pulling pain and a pulling sensation. This is because the skin and underlying tissue are under slight tension. The area around your ears may also be tender. This is normal and will subside after a few days.

    Recovery
In addition to post-operative pain, your body also needs time to recover. Initially, the treated area will be bruised, tender, and swollen. Immediately after the MACS lift, your scars may be red and raised. Don’t be alarmed by this—these are natural reactions of the body. Keep in mind that the swelling and bruising usually worsen during the first three days. This, too, will gradually subside afterward.

The skin will still feel firm for several weeks. This is due to the adhesion of the tissue layers and scar formation. Sometimes the skin may also be more or less sensitive. This is a normal reaction to the surgery and will gradually disappear.

  • A few weeks after the surgery, your face will still look strange to you. As the swelling and bruising subside, you’ll begin to see your youthful features more clearly. However, you’ll need to wait a few more weeks to get a truly accurate picture, since the healing process is gradual. In addition, minor changes may still occur in the months following the MACS lift.

Most patients are satisfied with the final result.

    Scars
Your scars will be located precisely along the hairline and a small section in front of the ear. At first, they will be red and swollen, but this will subside after a few weeks. It’s important to know that it can take one to one and a half years for your scars to fully settle. It is therefore important to keep them out of the sun during this period and to care for them properly with the scar cream we provided. Once the wound has closed and the scabs have fallen off, you may begin applying the cream. By applying it twice a day to the closed wound, you’ll increase the chances of a smooth healing process.

  • With plastic surgery procedures, the risks and likelihood of complications are low, since the patients are generally healthy individuals. However, please keep in mind that any surgical procedure can carry risks and complications.

At Boerhaave, we provide thorough information, a comprehensive medical history review, and a professional approach to minimize any risks as much as possible. You can also help by carefully following the instructions you received beforehand.

    Possible complications may include:

    • Hematoma (a collection of blood under the skin that requires removal)
    • Adverse reaction to the anesthesia
    • Postoperative bleeding
    • Infection
    • Changes in sensation
    • Permanent scarring
    • Changes in the hairline
    • Damage to underlying structures
    • Unsatisfactory aesthetic result.

    Smoking
Patients who smoke or use tobacco or nicotine products at the time of surgery are at a higher risk of complications, such as skin necrosis and impaired wound healing. It can also have a negative effect on anesthesia, potentially resulting in an increased risk of bleeding. It is important to stop smoking at least four weeks in advance and to continue abstaining throughout the entire recovery process.

People exposed to secondhand smoke may also be at an increased risk of similar complications compared to people with no exposure at all. The latter group has a significantly lower risk of complications.

Frequently Asked Questions

  • A MACS lift is the second step in the range of facelift variants and a more extensive treatment than the S-lift. Not only the skin, but also the underlying supporting tissue is returned to its original position, restoring the smoothness and contours of the face and lower jaw for a more youthful appearance.

  • A few weeks after the operation, as swelling and bruising subside, you will gradually see more of the youthful result; small changes may still occur over the following months. The scars lie along the hairline and a short distance in front of the ear, and can take up to a year and a half to settle fully.

  • Our plastic surgeon uses the tissue glue Artiss to glue different tissue layers together. Artiss contains substances that are important for blood clotting and act as a kind of glue. Artiss is applied in the form of a spray between the skin and the underlying tissue.

How does the tissue glue Artiss work?

In many plastic surgery procedures, the skin tissue is detached from the underlying tissue. After stitching, there is often space between the detached skin tissue and the underlying tissue. The tension on the skin and the stitches can stand in the way of the scars healing nicely. To prevent this, the plastic surgeon can glue the detached skin and the lower tissue layers together with Artiss. This puts less tension on the stitches. This can result in the scars healing more nicely. Advantages of Artiss tissue glue After a plastic surgery procedure, (secondary) bleeding can occur under the skin. This then becomes visible as bruising. Artiss prevents space from forming between the tissue layers by gluing them together. The risk of secondary bleeding and bruising is therefore lower. Because the tissue layers are glued together with Artiss, there is no room for wound fluid to accumulate and cause swelling. This promotes a faster recovery and a better result. With a facelift, for example, you will also be presentable sooner after the treatment. Because the skin adheres better and less wound fluid accumulates, a pressure bandage and drains may even be unnecessary with, for example, an S-lift , MACS lift and face and neck lift.

    Artiss therefore provides:

    • Less risk of secondary bleeding and bruising

    • Faster recovery

    • Less risk of swelling

    • Presentable sooner

    • Tension-free stitching of the incisions

    • Nicer scars.

Safety Artiss is a reliable product that is used as standard in many hospitals for all kinds of surgical procedures. It has been available for more than 30 years and its safety has been demonstrated in numerous scientific articles.
