About a endoscopic forehead lift

Aging can cause drooping eyebrows and deep frown lines or forehead wrinkles, which can make you look tired. A forehead lift lifts the forehead and eyebrows, giving you a younger, more open, and more rested appearance.
  • An open forehead lift costs €2,900, an endoscopic forehead lift €3,250. During a free, no-obligation consultation, our plastic surgeon discusses and assesses which procedure suits you best, including a personal cost estimate.
  • Yes, this is common. A forehead lift improves the position of the eyebrows, but does not correct excess skin on the eyelids themselves — an eyelid correction then provides a more complete result.
  • The swelling continues to increase for the first 3-4 days before it subsides; bruises usually fade within a week. Most people take 1 to 2 weeks off work.
  • At Boerhaave, a forehead lift is performed by experienced plastic surgeons with attention to safety, quality and a natural result. You can count on high-quality implants, personal guidance and comprehensive aftercare, so that you know where you stand throughout the entire treatment process.

Here's how the treatment works

Here you will find all the important information about this treatment, from the consultation and the treatment itself to follow-up care, the results, and potential risks.

  • Many people who are considering a forehead lift no longer recognize themselves in the mirror. Their facial expression looks tired, sullen, or aged, even though they actually feel energetic and full of vitality. If the position of your eyebrows, forehead, or upper eyelids makes you look less fresh or youthful, a forehead lift may be a suitable solution.

    Different Techniques: Open or Endoscopic

There are two commonly used techniques for a forehead lift: the open forehead lift and the endoscopic forehead lift. Which technique is most suitable depends on your skin type, the degree of sagging, and your personal preferences.

    Open Forehead Lift

In the classic method, a longer incision is made from ear to ear, just behind or within the hairline. This technique allows for a comprehensive lift of the skin across the entire forehead. The open forehead lift is often performed on people with significant excess skin or deep wrinkles.

    • Advantage: powerful and long-lasting results
    • Disadvantage: longer recovery time and a larger scar, although this is usually well-hidden in the hair and not suitable for those with a significantly receding hairline.
    Endoscopic Forehead Lift

The endoscopic technique is less invasive and involves making a few small incisions in the scalp. Using a small camera (endoscope) and special instruments, the forehead is lifted via a minimally invasive approach.

    • Advantage: shorter recovery time, minimal scarring
    • Disadvantage: less suitable for cases with significant excess skin and not suitable for a long forehead and/or receding hairline.

If you have a long forehead and/or a significantly receding hairline, neither the open nor the endoscopic forehead lift is usually suitable. In that case, a eyebrow lift may be a better alternative. A plastic surgeon can assess this during a free, no-obligation consultation.

    Combination with Eyelid Surgery

Although a forehead lift improves the position of the eyebrows and thus makes the eyes look more refreshed, it does not correct excess skin on the upper eyelids themselves. If you have drooping eyelids, a forehead lift can be combined with an upper eyelid lift for a more complete result.

  • Plastic Surgeon
Your consultation with us will always be with the plastic surgeon who will be treating you. During this consultation, you can explain what bothers you most about the position of your eyebrows and what you would like to change. The surgeon will review your medical history and examine your face. During this examination, the bone structure, underlying skin structure, and the thickness, texture, and elasticity of your skin will be assessed. Based on this, the surgeon will recommend the surgical technique best suited for you and, together with you in front of a mirror, explain what this technique entails and what you can expect.

Afterward, everything will be summarized in a treatment plan, and the total costs will be discussed with you immediately. The purpose of the consultation is to provide you with information that is as clear and comprehensive as possible.

    Additional Treatment
In some cases, the plastic surgeon may recommend additional surgery. An endoscopic forehead lift can be combined with, for example, an eyelid correction, resulting in a more balanced facial appearance. Any additional procedures, such as a facelift, can also be performed at the same time.

    Consultant
Following your consultation, you will have a meeting with your consultant. The purpose of this meeting is to inform you about the general procedures that need to be arranged in connection with the surgery.

  • Depending on the treatment you choose, please note that the surgery itself will take between one and one and a half hours. Afterward, you will remain with us for a few more hours before being discharged home under supervision. This is because an endoscopic forehead lift is always performed under general anesthesia.

Before the surgery, your plastic surgeon will review the entire procedure with you once more. He will then mark your forehead and eyebrows according to the surgical plan. After you have been anesthetized, three to five small incisions will be made in the scalp. Through one of these incisions, the surgeon inserts an endoscope—which contains a small camera—and a dissection scoop through another incision. The entire procedure is performed using the image displayed on a monitor.

Thanks to advanced equipment, the skin of the forehead is separated from the underlying tissue. This makes it possible to lift the eyebrows or smooth out the frown lines. Furthermore, this technique keeps the incision area extremely small and accelerates the healing process. At the end of the procedure, your skin is secured using tissue adhesive and sutured without tension. This ensures that your eyebrows remain in the correct position during the healing phase. This not only promotes recovery but also ensures a better and longer-lasting result.

  • After the surgery, you will experience some swelling and bruising. You can speed up the natural healing process by cooling your face for the first two days and elevating your head with an extra pillow while in bed at night. Additionally, the swelling and bruising will worsen over the first three days before gradually subsiding.

    Recovery
As with any surgical procedure, your body needs time to recover. You’ll experience swelling, and fluid may accumulate around your eyelids. The skin on your forehead may still feel tight for a few weeks. This is due to the tissue layers fusing together and scar formation. Don’t be alarmed by this; it’s a natural reaction of the body. Your stitches will dissolve on their own, and the swelling and bruising will gradually subside.

    Scar Cream
During your first follow-up visit, you’ll be given a scar care cream to take home. Once the wound has closed and the scabs have fallen off, you may begin applying the scar cream. Applying this to the closed wound twice a day will increase the likelihood of a smooth, even healing.

  • The healing process following your surgery will be gradual. As a result, you’ll need to wait a few weeks to get a good idea of the final result. However, minor changes may still occur in the months following the procedure. You can expect to see the final, clear results in about six months.

Most people who undergo a forehead lift are surprised and satisfied with the results, especially because it will give you an open and refreshed appearance. Your tired and possibly stern appearance will be a thing of the past. Especially for younger people with a low forehead or deep frown lines, it can help reduce feelings of embarrassment or shyness.

Keep in mind that gravity and the ongoing aging process may still take their toll in the years following the treatment. However, forehead lines and frown lines will no longer become as pronounced over the years. It therefore remains important to protect your face from the sun and maintain a healthy lifestyle.

  • It is important to know that any surgical procedure can involve risks and complications. Plastic surgery generally involves healthy individuals, and the risks and likelihood of complications are low. Through thorough counseling, a comprehensive medical history, and our professional approach, we minimize these risks to an absolute minimum.

In addition, you can help reduce certain risks by carefully following the instructions you have been given.

    Some possible complications may include:

    • Hematoma (a collection of blood under the skin that requires removal)
    • Adverse reaction to the anesthesia
    • Postoperative bleeding
    • Infection
    • Changes in sensation
    • Permanent scarring
    • Damage to underlying structures
    • Unsatisfactory aesthetic result
    • Uneven eyebrow alignment.

    Smoking
Patients who smoke or use tobacco or nicotine products at the time of surgery are at a higher risk of complications, such as skin necrosis and impaired wound healing. Smoking can also have a negative effect on anesthesia, potentially resulting in an increased risk of bleeding. It is important to refrain from smoking for at least four weeks before surgery and to continue abstaining throughout the entire recovery process.

People exposed to secondhand smoke may also be at increased risk for similar complications. People who are not exposed have a significantly lower risk of such complications.

Frequently Asked Questions

  • There is a lot of confusion surrounding the terms used for a facelift. Below you will find an explanation of the three facelift variants that are possible and what you can expect from them.

    S-lift

The least invasive facelift is the S-lift (sometimes also called a mini facelift or mini lift). A small S-shaped incision is made in the skin, running in front of the ear. After a limited area of the skin has been detached from the underlying tissue, the skin is pulled towards the ear so that the jawline regains its original contour. A strip of skin in the shape of an S is removed, after which the skin is stitched. An S-lift only produces results for the area just above the jawline. An S-lift can be used to correct so-called jowls. The contour of the jawline becomes tighter, but wrinkles around the mouth, the lips and the eyes do not disappear after an S-lift. With the S-lift, only the superficial skin tissue is lifted, not the underlying muscle tissue. An S-lift is performed as a day case and can take place under local anaesthetic. If you prefer, you can also opt for an S-lift under general anaesthesia. As a rule, you can go home the same day after the treatment.

    Facelift MACS lift

Unlike the superficial skin correction of the S-lift, a MACS lift tightens not only the facial skin but also the deeper tissue layers. This so-called SMAS tissue (SMAS: Superficial Muscular Aponeurotic System) is heavier than the skin itself and can cause the upper tissue layers (skin and fatty tissue) to sag. In a MACS lift, the plastic surgeon makes an incision from the temple in the hairline downwards, running in front of the outer ear. After the skin and muscle tissue have been detached from the underlying tissue layer, the facial muscles on both sides are first tightened in the direction of the ear and the temple. The plastic surgeon stitches this muscle tissue in such a way that it returns to its original position. This is also known as plication. The muscles in the face are shortened by folding them double and stitching them in place. The overlying skin is tightened in the same direction. Excess skin tissue is removed and the skin is closed again with stitches at the site of the incision. Because a MACS lift also lifts the deeper tissue layers, it is a much longer-lasting solution than an S-lift. A facelift can remove deep folds around the mouth and so-called jowls. A MACS lift always takes place as a day case. This means that, as a rule, you can go home the same day.

    Face/neck lift

The combined face and neck lift is the most extensive facelift and also gives the most comprehensive results. As with a MACS lift, the plastic surgeon makes an incision from the temples downwards, running in front of the ear. The incision runs under the earlobe, back upwards, and ends at the hairline behind the ear. This makes it possible to tighten the tissue of the neck as well. As a result, a face and neck lift produces results both in the face and in the area below the jawline. A face and neck lift delivers the most comprehensive results. Deep folds in the face and in the neck can disappear with a combined neck and facelift. An eyelid correction can also be performed in addition to a face and neck lift. A face and neck lift always takes place under general anaesthesia and always includes an overnight stay. In most cases, you can go home again the following morning.

    Neck lift

For excess sagging skin on the neck below the chin, a so-called turkey neck, a neck lift (also known as a Z-plasty) is a good solution. By making a small incision (in the shape of a Z) just below the chin, the skin can be tightened. This treatment only produces results for the neck and leaves a visible scar. People who suffer from sagging skin under the chin (the so-called turkey neck) choose this treatment. Only in a few cases does a neck lift give the desired result. Usually the neck involves not only slack skin but also slack neck muscles. In that case, it is advisable to undergo a combined neck and facelift. In this procedure, the skin and the underlying muscle tissue are tightened from behind the ear, giving results for the whole neck. A neck lift (Z-plasty) can be performed under local anaesthetic and the procedure takes place as a day case. This means that, as a rule, you can go home the same day. Combined neck and facelift

    Forehead lift

A facelift cannot remove wrinkles on the forehead. A forehead lift is a solution for that. Both a facelift and an (endoscopic) forehead lift can also be performed in combination.

  • Most people experience little or no pain after the treatment. If the pain or discomfort persists, it can be easily relieved with a painkiller such as paracetamol.
  • You will benefit from the result of your endoscopic forehead lift for years. However, you should realise that the ageing process simply continues. The new position of the forehead is permanent. That is an advantage you will always keep.
  • During a no-obligation consultation, the plastic surgeon assesses which technique suits you best. This involves looking at your skin structure and elasticity, the underlying bone structure, the degree of sagging, and the length of your forehead and hairline. With a long forehead and/or a strongly receding hairline, the forehead lift techniques are often less suitable, and an eyebrow lift, for example, is considered as an alternative. Your own wishes also play an important role. Everything is summarised in a personal treatment plan, in which the costs are also discussed straight away.
